Question: I am interested in learning more about St. John Home. What do I do?

Answer: Thank you for your interest! Please call St. John Home at 612-245-3474 to speak with a staff member, complete a phone interview, and set up an in-house visit and/or admission assessment. We look forward to hearing from you!

 

 

 

Question: What makes St. John Home different from other Assisted Living Facilities?

Answer: St. John Home is owned and operated by a registered nurse with both nursing home and acute care hospital experience. This allows for a higher level of care for patients with more complex physical and mental needs. Our staff-to-resident ratio is 2:1. We also offer skilled nursing services personalized to the individual needs of each resident.

 

 

 

Question: What if I cannot pay out of pocket for the care of my loved one?

Answer: St. John Home accept Elderly, CADI, and BI Waiver/Medical Assistance for residents who qualify (e.i. government funding that helps low-income families pay for their care—to see if you qualify, each family must individually contact Hennepin County Long Term Consultation Intake at 612-348-4500). For other questions about coverage, please contact your insurance company directly.

 

 

 

Question: Can my loved one still use his or her current physician at St. John Home?

Answer: Yes, but we encourage residents to use a local physician near the metro area, in the event of immediate needs requiring treatment or consultation from a nearby medical facility.

 

 

 

Question: What is the current licensure for St. John Home?

Answer: St. John Home is licensed by the Minnesota Department of Health as an Assisted Living Facility and DHS Customized Living.

 

 

 

Question: Does St. John Home have policies, procedures, and staff training?

Answer: Yes, we update all of our staff on current policies and procedures and provide training on necessary resident care topics.

 

 

Question: What are the new changes that the State Legislature made regarding Minnesota Residents when choosing a care facility?
